Introduction
As a YSU student, faculty, or staff member and as a user of YSU computer-related services, you should use this form to submit the following types of requests:
- computer-related assistance or information
- new equipment, software, or permissions
- any change to existing equipment or accounts
A service request is defined as “a user request for information or advice, or for a standard change.” Some examples of an appropriate service request include computer or printer purchases and general information requests.

Please provide the following information:
- Building and room number where the service is needed
- YSU Technology Services (TS) tag number of the device (if applicable)
- Contact information (phone number preferred)
- Any additional information or files that you would like to include as an attachment
