IT General Request

Introduction

As a YSU student, faculty, or staff member and as a user of YSU computer-related services, you should use this form to submit the following types of requests:

  • computer-related assistance or information
  • new equipment, software, or permissions
  • any change to existing equipment or accounts

A service request is defined as “a user request for information or advice, or for a standard change.”  Some examples of an appropriate service request include computer or printer purchases and general information requests.

Please provide the following information:

  • Building and room number where the service is needed
  • YSU Technology Services (TS) tag number of the device (if applicable)
  • Contact information (phone number preferred)
  • Any additional information or files that you would like to include as an attachment
