Forms: QR Code to Form

Generating a QR code to your form is as easy as sharing the link. This article walks you through the steps of obtaining the QR code to your form.

How can I generate a QR code for people to fill out my form?

You can distribute a QR code to your form as easily as you can share the link. QR codes are beneficial for sharing on flyers, posting to digital signage, table toppers, incorporating in presentations, etc. 

To get a QR code to your form, take the following steps:

1. Create a new form or open an existing form.

2. Click on the Collect responses button at the top right.

3. On the Send and collect responses menu, select the QR Code button (3rd from the left). At the QR code screen, click the Download button. The QR code will download as an image file to your downloads folder or your default specified download location. From that location, you can import the QR code image file into your program of choice for integration into your publication, info graphic, document, presentation, etc. 

Note: The permissions to the form will be the same for the QR code as the link. For example, on the left side, with the setting Only people in Youngstown State University with a link can respond will require @ysu.edu sign in before proceeding.
